What's special about Accountable Health Networks?

Accountable Health Networks (AHN) are a new kind of network, one in which your primary doctor (also called your primary care provider, or PCP) acts as your quarterback—the point person who leads your care team to manage your total care.

For you, this means:

Coordination of care

Your team can share information to make sure they're all on the same page. This approach can help them see the bigger picture of your health and any gaps in your care.

Fewer tests

All of your doctors have access to any test results requested by one of them. Removing redundancy can save you money.

Direct access to specialists

Every specialist in your AHN is already part of your team so referrals aren't needed.

Everyone in your family can choose a PCP

Each person has individual needs and can choose a PCP based on their special concerns.

How does an AHN work?

For a person with type 2 diabetes who needs help from multiple providers, it's important that all agree on the care plan. The patient would choose a PCP to be point person—to keep the other providers informed about tests, prescriptions, medical supplies—and coordinate with specialists on managing care.

This means better communication, which could lead to great care at a lower price.

How to find a PCP

It takes just three steps to find a PCP:

  1. Sign in to BridgeSpan.com.
  2. Go to Find a doctor.
  3. Use the search box to find a doctor by name or specialty

Each member of your family can choose a different PCP, and you can change as often as you'd like.

Tell BridgeSpan your PCP choice

We include your PCP's name on your member ID card, so it's important for you to let us know who will be managing your care. If you change your PCP, you'll receive a new member ID card that shows the new name. Let us know who your PCP is in one of the following ways:

  • Fill out the online form.
  • Call us at 1 (855) 857-9944.
  • Email us at cs@bridgespanhealth.com.
  • Go to Message Center, compose a new message and select Change my primary care provider as the topic.
