Accountable Health Networks (AHN) are a new kind of network, one in which your primary doctor (also called your primary care provider, or PCP) acts as your quarterback—the point person who leads your care team to manage your total care.

A PCP gets to know you to help you make the best decisions about your health. You can choose a PCP based on a specialty or even location, so that getting care is easier and more convenient for you.

We include your PCP's name on your member ID card, so it's important for you to let us know who will be managing your care. If you change your PCP, you'll receive a new member ID card that shows the new name. Let us know who your PCP is in one of the following ways:
- Fill out the online form.
- Call us at 1 (855) 857-9944.
- Email us at firstname.lastname@example.org.
- Go to Message Center, compose a new message and select Change my primary care provider as the topic.
